They come in a report from the Police Conduct Authority into the handling of what's become known as the Kawerau siege, two years ago.
Four police officers were shot when they entered the home of gunman Rhys Warren.
The report says the decision to enter the home put their lives in danger and should never have been made.
Assistant Commissioner Bill Searle says police couldn't use dogs on the day, as broken glass from smashed windows was littered throughout the house.
They've now issued dogs with leather boots for such situations and are trialling a remote camera.
All four wounded police officers have returned to work but Searle says one is still suffering on-going issues.
The 22-hour siege ended peacefully and Rhys Warren was later sentenced to preventive detention on six charges, including two of attempted murder.
